Thoracic aortic endografts are deployed through large sheaths (20-24F). Traditionally, access requires open arterial exposure. Percutaneous access is feasible in selected patients. We have performed 21 endovascular thoracic aortic repairs using entirely percutaneous techniques with low complication rates. Appropriate patient and graft selection, deployment tips, and potential complications are discussed.
