Ethnic Differences in Infrarenal Aortic Diameter in a Population-Based Study

Univ of Texas Southwestern Med Ctr, Plano, TX, USA. Objective: Abdominal aortic aneurysms are defined as a 50% or greater increase in infrarenal aortic diameter (IAD). However, normal IAD has not been defined for all ethnic groups as minorities have been underrepresented in most studies. To assess ethnic differences in IAD and the effects of age, gender and body size, we analyzed a population-based probability sample.
Methods: Participants (2,515) in a population based study underwent high-resolution magnetic resonance imaging (MRI) of the abdominal aorta. Analyses of variance and multiple regression analyses were used to assess the relationship between race/ethnicity, age, gender and body size to IAD. Subjects with AAA detected by MRI (defined as IAD ≥ 3.0 cm) were excluded from the analysis.
Results: Increasing age, height, weight, body mass index, and body surface area as well as male gender and black race were independently associated with higher IAD by multivariate linear regression (all
P < 0.001). Male sex was and black race were associated with 0.17 cm and 0.008 cm increases in IAD, respectively. Although whites and Hispanics had similar mean IAD (
P =.7), weight, body mass index and body surface area were significantly larger among Hispanics (all
P < 0.001). In age-, gender- and body size-adjusted
linear and logistic regression analyses, Hispanic ethnicity was associated with a 0.14 cm reduction in IAD.
Conclusions: Ethnic differences exist in infrarenal aortic diameter. Body size, in particularly, has significant effects on IAD among Hispanics and is associated with increased IAD in this ethnic group in the general population.
